Chorus Community Health Plans (CCHP), an affiliate of Children’s Wisconsin, offers affordable, high-quality health coverage to individuals and families across the state.
We bring together trusted clinical expertise and community partnerships that support whole-person health and long-term wellness. Our goal is simple: give members the tools, support and resources they need to take charge of their health and wellness.
Chorus Community Health Plans currently serves more than 100,000 adults and children across 68 Wisconsin counties. With the combined strength of Children’s Wisconsin and local partners, we support healthier families and stronger communities.
To be eligible for Chorus Community Health Plans (CCHP) insurance, you must be enrolled in the BadgerCare Plus Medicaid Program.
