Partner with Us
If you would like to join our CCHP network as a contracted provider, start by submitting a Letter of Interest. This step begins our contracting review and helps us understand your services. If approved, you’ll receive a Provider Network Agreement form mailed to you along with detailed credentialing information in the coming weeks* of receiving your submittal.
Applicants must be certified with Wisconsin Medicaid
*Please allow 30 days for review of your letter of interest.
Provider Responsibilities
CCHP offers the support, resources, and education providers need to ensure they are in compliance with our policies as well as the state’s policies. The provider is responsible to follow these policies. It’s the primary care physician’s (PCP) responsibility to ensure coordination and continuity of care with providers, including all behavioral health and long-term care providers. PCPs are also responsible to make sure that our members receive preventive care appropriate for their age group.
