Helping You Stay Healthy
Care Management Programs at Chorus Community Health Plans (CCHP) help our members and their families build healthy habits. These free services are especially helpful for those living with chronic or complex conditions.
Members receive education, digital tools and personalized support that encourage healthy habits. Members can also work one-on-one with a care manager who understands their goals. All members who qualify can begin using these services right away.
Specialty Programs and Support
CCHP offers several specialty programs designed to support members with unique health needs. Each program provides simple guidance and personalized tools. All services stay confidential and are tailored to your specific situation. A few of our programs include:
- Asthma Program: Education, environmental trigger management, and action planning support for kids.
- Behavioral Health Program: Coordination with mental health providers and help navigating available services.
- Diabetes Program: Nutrition tips, activity planning, and blood sugar tracking for adults.
- Pregnancy and Newborn Program: Guidance throughout your pregnancy, including postpartum and newborn care.
- Social Navigation: Support with everyday challenges that can affect well-being—like housing, food, transportation and financial stress.
Enrolling
Eligible members can join a health program at no cost. If you qualify for a program, we enroll you automatically. You’ll receive a text, email or letter with helpful details. Our team will guide you through each step, so getting started is simple.
Earning Points
You can earn points and redeem them for gift cards to support your wellness journey. Earn points by:
- Completing healthy activities through the Chorus Wellness Portal.
- Completing an annual Health Needs Survey. This survey also helps us better understand your care needs.

We also offer extra support for members with more serious health or life challenges.
This includes people who:
- Are going through a high-risk pregnancy
- Have had multiple emergency department visits or hospitalization for asthma or diabetes
- Have more than one ongoing illness
- Struggle with mental health
- Had a serious injury
We may find out you need help by looking at your health records or hearing from your doctor or others in your community. If we think we can help, someone from our care team may call you.
