At Chorus Community Health Plans (CCHP), we know life can be hard sometimes. You might be dealing with things like anxiety, depression, ADHD, bipolar disorder, schizophrenia, or other mental health concerns. You’re not alone—and we’re here to help. If you're feeling overwhelmed, struggling with substance use, or just need someone to talk to, our Care Management Team is here to support you.
What Mental Health Challenges Can Look Like
Everyone’s experience is different. You might notice:
- Feeling sad or losing interest in things you enjoy
- Body aches or pains with no clear reason
- Feeling angry, worried, or forgetful
- Trouble with work, school, or caring for others
- Using alcohol, tobacco, or drugs more than usual
- Big mood swings
- Thoughts of hurting yourself or others
- Problems with family or friends
Mental health challenges can also make other health conditions—like diabetes, heart disease, or cancer—harder to manage.

- Support you after a hospital or ER visit
- Offer extra help when you need it
- Talk with you about substance use and help with referrals
We work with you—and your care team—to make sure you have the tools and support to feel your best.
Medication-Free Treatment Options
Not everyone wants or needs medication to feel better. CCHP offers a Medication-Free Treatment Program that may include:
- Counseling or therapy
- Education and self-help tools
- Healthy lifestyle tips
- Peer support and community resources
Recovery takes time, and everyone’s path is different. We’ll help you find what works best for you.
