We are proud to offer an exclusive wellness portal that makes managing and maintaining a healthier lifestyle simple and convenient. Our plans offer a unique portal experience for our members. This portal allows you to learn about your health, complete activities and get incentives.

Here are few examples of wellness activities you can complete to earn incentive points:

  • Health needs survey
  • Wellness visit
  • Diabetes-related wellness activities
  • Postpartum visit

Visit your Wellness Portal account to learn about additional wellness activities and to redeem your reward points. Points can be redeemed for either physical or electronic gift cards from a variety of retailers.

Please note that reward points expire 6 months after they are earned. Check your Wellness Portal account regularly to monitor the status of your points.

The Wellness Portal is an interactive, online tool where you can:

  • learn about your health.
  • explore healthy lifestyle tips.
  • find upcoming CCHP classes and community events.
  • get rewards for completing wellness activities.

All CCHP members can use this tool. Register here!

You will need:

  • your member ID number.
  • a valid email address.

Note: A valid email address is necessary to receive notifications about earned rewards.

Member ID numbers are listed on the front of both member CCHP and ForwardHealth insurance cards.

  • If you do not have your CCHP insurance card, please call 1-800-482-8010 to obtain your member ID card.
  • If you do not have your ForwardHealth insurance card, please call 1-800-362-3002 to obtain your member ID card.

Members can earn reward points for completing wellness activities. Points expire 6 months after they are earned. Check your Wellness Portal account and email for updates about your points’ status.

Reward points can be redeemed for physical or electronic gift cards from vendors such as Walmart, Target, Amazon and many more.

After completing a wellness activity, you will receive an email about available reward points. Log into your Wellness Portal account, locate the 'Rewards' tab, and click on the 'Redeem' link. If you would like to receive an electronic gift card but do not have access to a smart phone, computer, or printer you can call the Wellness Portal line at 414-266-3636 to request an electronic gift card to be printed and mailed to you.

No. Only current CCHP members can access the Wellness Portal and redeem their points. For this reason, you should redeem your points when they become available.
