Clinical and Preventative Guidelines
At Chorus Community Health Plans (CCHP) our Clinical and Preventative Guidelines assist clinicians by providing a framework for the evaluation and treatment of patients. These guidelines have not been developed as, nor should they be construed as absolute standards or minimal levels of care. The physician, in light of the individual circumstances presented by the patient, must make the ultimate judgment regarding the application of any specific mode of treatment. In other words, these guidelines are not intended to replace the clinical judgment of health care professionals.
CCHP's Medical Advisory Committee reviews the following guidelines at least every two years or earlier if there are significant evidence-based changes in best practices or updates to reference guidelines.
Immunization Schedule Guidelines
Stay current.
The Wisconsin Department of Health Services (DHS) provides immunization schedules recommended by the American Academy of Pediatrics (AAP) and the American Academy of Family Physicians (AAFP). These guidelines help ensure that children and adults receive the right vaccines at the right time.

The Vaccines for Children (VFC) program helps make sure kids get the protection they need. Children 18 years old and younger can receive free or low‑cost vaccines if they’re enrolled in Medicaid. CCHP members—whether through BadgerCare Plus or Care4Kids—are eligible to get their routine vaccines through the VFC program.
