We appreciate the trust you placed in Chorus Community Health Plans (CCHP) throughout your time as our member. Even though our Individual and Family Plans ended on January 1, 2026, we want to ensure you still can access the documents, forms, and information you may need. Below you’ll find additional resources for our past members.

ONLINE MEMBER PORTAL ACCESS NO LONGER AVAILABLE

As of 1/1/26 the IFP member portal is no longer accessible. Past members may contact Customer Service at 844-201-4672 to discuss past plan information or obtain claim status. Please note, you will be mailed an Explanation of Benefits once claims are processed.
