Individual and Family Plans No Longer Offered
Effective January 1, 2026, Chorus Community Health Plans officially exited the Health Insurance Marketplace. Marketplace plans, including plans sold off Marketplace, are no longer available or active.
While our Individual and Family Plan products have ended, we remain committed to ensuring past members, providers, and brokers can still access the information and resources they may need.
