Healthy Mom, Healthy Baby Program
Our Healthy Mom, Healthy Baby program is a free benefit for Chorus Community Health Plans (CCHP) members that offers a personalized approach through case management during all stages of your pregnancy. Whether this is your first pregnancy or you have other children, we want to support you in reaching your goals, making informed health care choices and having the healthiest pregnancy.
When you participate in the program, you'll partner with a CCHP case manager who can help with:
- Getting necessary health care and support
- Connecting to resources in the community
- Health education materials before and after the baby is born
- Personal support for issues that matter to you most
We also have a nurse practitioner/midwife who is part of CCHP’s Healthy Mom, Healthy Baby program. She can meet with you during or after your pregnancy and is available to help you with services in addition to your regular OB provider. She can answer questions, provide breastfeeding support, birth control counseling, and provide an initial prenatal or postpartum checkup over the phone or in your home until you are able to see your regular doctor.
