Advance Directives
Chorus Community Health Plans (CCHP) members have the right to make decisions about their health care, including the right to plan for care in the future if they become unable to communicate their wishes. This planning is done through an Advance Directive.What Is an Advance Directive?
An Advance Directive is a legal document that lets you express your health care wishes in advance.
It may include:
- A Power of Attorney for Health Care, naming someone you trust to make medical decisions if you are unable to do so.
- A Living Will (Declaration to Physicians), stating your preferences about medical treatment if you are terminally ill or permanently unconscious.
These documents are recognized under Wisconsin law and help ensure your choices are known and respected.
Why It’s Important:
Creating an Advance Directive helps:- Protect your right to make your own medical decisions.
- Guide your doctors and family members if you can’t speak for yourself.
How to Create an Advance Directive:
You can talk with your primary care provider to learn more about Advance Directives and get help completing one.CCHP encourages all members 18 years and older to have an Advance Directive and share a copy with their doctor.
