Vaccines, also called immunizations, help keep you and your family healthy. It’s normal to have questions about them, most people do. You might be learning about vaccines for the first time, or you may be catching up now, and that’s okay. Chorus Community Health Plans (CCHP) is here to support you.
Talk to Your Primary Care Provider
Your primary care provider (PCP) is the best person to talk to about vaccines. They know your health history and can explain which vaccines you may need. They can also answer any questions you have, big or small. Need help finding a PCP? Use our Find a Doctor search tool to find care near home.
It’s Okay to Ask Questions
Most people have questions about vaccines. You might wonder how they work, if they are safe or when you should get them. Asking questions is a smart way to take care of your health.

Check Your Vaccine Records
Not sure which vaccines you or your child have had? You can look them up in the Wisconsin Immunization Registry (WIR). It’s easy to use and helps you see what you might need next. Your PCP can also look at your WIR record and help you understand it.
Vaccines for Children
One of the best ways to keep your child healthy is by getting all recommended vaccines. Vaccines help protect your child, your family, and your community from serious illnesses.
The best place for your child to get their vaccines, and for you to ask questions, is at their HealthCheck appointment. Your child’s PCP can explain which vaccines are recommended at each age and why they matter. They can also talk with you about any worries or questions you may have. It’s completely okay to ask questions. Many parents do.
HealthCheck appointments are covered for all Medicaid members from birth through age 20. These visits include a full check‑up, time to talk with your child’s provider and any vaccines your child may need. If your child is behind on vaccines or you’re just starting now, that’s okay. Your provider can help you make a plan to get back on track.

Commonly Asked Questions
Learn more about which vaccines you and your family members are due for by:
- Calling your PCP to discuss which vaccines you and your family members should get
- Checking online to see which vaccines you and your family have already received. You can also see which vaccines are next. The Wisconsin Immunization Registry (WIR) webpage offers a Public Immunization Record Access portal. It is free to use and does not require registration.

You can find out which vaccines you and your family have already gotten by:
- Checking your Wisconsin Immunization Registry (WIR) vaccine record. The WIR portal webpage is available in English, Spanish, and Hmong
- Ask your doctor if they enter vaccine records to WIR. Some doctors do not. If your doctor doesn’t enter vaccine records to WIR, ask for a copy of your vaccine record. You can take this record to your local health department office. They can enter the information into WIR for you.
Helpful tip: whenever you get a vaccine, always ask for an updated copy of your vaccine record.
