Referrals

Referrals serve as a pivotal step when a patient's health condition requires specialized expertise or services beyond the primary provider's scope. Referrals help ensure that patients receive the most appropriate care by connecting them with the right specialists or resources within the healthcare system. Through referrals, patients can access the tailored care required to address their unique medical needs.

In-network:

• CCHP does not require written referrals for its members to any in-network provider.

Out-of-network:

• Providers must call CCHP’s Clinical Services department at 877-227-1142 to request CCHP’s Authorization Request Form.
• Non-Urgent Request: CCHP will notify the provider of the approval or denied decision within 14 days after receiving all necessary information.
• Urgent Request: CCHP will notify the provider of the approval or denied decision within 72 hours after receiving all necessary information.
