If you are interested in joining our network as a contracted provider, you can begin by submitting a Letter of Interest form. If approved, we will mail you a Provider Network Agreement form, along with credentialing information within a few weeks of receiving your submittal.

Please allow 30 days for review of your letter of interest.
CHIROPRACTORS: If you would like to become a contracted chiropractic provider with CCHP, please call Wisconsin Health Choice, LLC at 262-201-4327. Wisconsin Health Choice, LLC administers the chiropractic network for CCHP and will assist you.
DENTAL PROVIDERS FOR BADGERCARE PLUS: If you would like to become a contracted dental provider for CCHP BadgerCare Plus members, please contact Dental Professionals of Wisconsin at 414-389-9880. Dental Professionals of Wisconsin administers the dental network for CCHP and will assist you.
Our Credentialing Process
All fully licensed practitioners must go through the credentialing process upon notification that their Letter of Interest has been received and approved — whether they’re in the process of contracting with Chorus Community Health Plans or are already contracted and adding staff.
Getting Started
For BadgerCare Plus Providers
If you would like to become a contracted BadgerCare Plus provider with CCHP, please complete the Letter of Interest first and review our credentialing instructions on how to submit an application. We only accept applicants who are Medicaid certified.
For Individual and Family Plan Providers
If you would like to become a contracted provider with CCHP, please complete the Letter of Interest Form with CCHP first and review our credentialing instructions on how to submit an application.
