Healthy Mom, Healthy Baby Incentives
We want to reward you for taking care of yourself!
Life gets busy – especially when you are pregnant.
You can earn over $100 in incentives when you:
- Join our Healthy Mom, Healthy Baby program
- Go to a doctor’s visit in your first 12 weeks of pregnancy
- Go to your postpartum visit after having your baby

Redeem your incentives through our Wellness Portal, which has gift cards to a variety of stores. These are great for baby needs, treating yourself or helping make life’s costs a little bit easier.
First Trimester Prenatal Visit Incentive
Qualify for a $25 incentive when you have a doctor's visit in your first trimester.
The details:
1) Attend an appointment with your doctor (OB or nurse midwife) in your first 12 weeks of pregnancy.
2) Have your doctor verify your appointment by completing the incentive card (this is in your prenatal mailed packet or download here.)
3) Return the incentive card to us before you are 18 weeks pregnant. You may mail or fax the card to us. Your doctor's office can also send in the card for you.
4) After verifying, we will enter your points into the Wellness Portal for you. The points are typically available for you within 2 weeks from when we receive your completed incentive card.
Can't get in to see your doctor in your first 12 weeks of pregnancy?
You can still receive this incentive! Our CCHP nurse midwife can meet with you in-person or over the phone, until you are able to see your own doctor. Simply call us at (414) 337-BABY (2229) to schedule an appointment with our nurse practitioner.
Saw your doctor in your first trimester but forgot to have the incentive card filled out?
We may still be able to grant the incentive if we can verify the appointment. Call or email us with the details of your appointment and we will try to assist you!
Postpartum Visit Incentive
Qualify for a $40 incentive when you attend your postpartum visit.
The details:
1) Attend a postpartum appointment with your doctor (OB or nurse midwife) 7-84 days after you have your baby.
2) Have your doctor (OB or midwife) verify your appointment by completing the incentive card (this is in your post partum mailed packet or download here.)
3) Return the incentive card to us by mail or fax. Your doctor's office can also send in the card for you.
4) After verifying, we will enter your points into the Wellness Portal for you. The points are typically available for you within 2 weeks from when we receive your completed incentive card.
Having a difficult time seeing your doctor for a postpartum visit?
If you are having a difficult time getting back to see your doctor for your postpartum visit, or if you would like additional support, our nurse midwife would be happy to meet with you! Simply call us at (414) 337-BABY (2229) to schedule an appointment with our nurse practitioner.

Additional Healthy Mom, Healthy Baby Incentives
Qualify for up to $50 when you participate in - and complete - the Healthy Mom, Healthy Baby program with one of our CCHP case managers. Participation typically includes regular phone calls (or in-person visits) spread out over your pregnancy through your first 6-12 weeks after delivery (or longer if you prefer) as you work on your health goals and transition to life with your new baby.
The details:
1) Agree to enroll in the Healthy Mom, Healthy Baby program.
2) Connect with your case manager at the agreed-upon times.
3) Work on health goals you identify with your case manager.
4) Once you have delivered and attended your postpartum visit, you and your case manager will discuss a program completion date.
5) Your case manager will submit your incentive for the CCHP Wellness Portal.
