Z codes notate social and economic factors that contribute greatly to our members’ health including:

  • High BMI
  • Tobacco abuse
  • Drug abuse
  • Alcohol abuse
  • Psychosocial circumstances
  • Homelessness
  • Family conflict
  • Employment issues

By providers notating on claims these social determinants of health through Z codes, CCHP community health navigators and case managers can use these codes to assess resource needs. Please include these ICD-10 Z codes on claims if you notice members that may need additional support. CCHP is always looking for ways to help improve the lives of our members and by you notating these codes this gives our team more opportunities.
