Outreach and Educational Programs
Chorus Community Health Plans (CCHP) wants our members to receive the right health programs at the right time, in the right place. To help make this happen, we offer outreach and educational programs to support our members to engage in a healthy lifestyle.
CCHP offers dedicated Case Management services to members with challenging health and social conditions that prevent them from reaching their optimal level of health and wellness. The program is free, voluntary and member-centered. To learn more about these resources, please call (414) 266-3173.
