Outreach and Educational Programs
Chorus Community Health Plans (CCHP) wants to help our members stay healthy.
We offer free programs to help you and your family live a healthy life. These programs include helpful information, support, and incentives. Read more about them in your member handbook or by visiting the links below.
How It Works
If you qualify for a program, you will be signed up automatically. You then may get a text, email, or letter with more information. You can also earn points for doing healthy activities. These points can be used for gift cards on the Chorus Wellness Portal.

Additionally, all members are eligible to earn points once a year for completing a Health Needs Survey.
If you want help with your diabetes or pregnancy, or to opt out of participation in the incentive programs, call us at 414-266-3173 or email healthpromotion@chorushealthplans.org.
We also offer extra support for members with more serious health or life challenges.
This includes people who:
- Are going through a high-risk pregnancy
- Have had multiple emergency department visits or hospitalization for asthma or diabetes
- Have more than one ongoing illness
- Struggle with mental health
- Had a serious injury
We may find out you need help by looking at your health records or hearing from your doctor or others in your community. If we think we can help, someone from our care team may call you.
