BadgerCare Plus Referrals
In-network: CCHP does not require written referrals for its members to any in-network provider.
Out-of-network:
- Providers must call CCHP’s Clinical Services department at 877-227-1142 to request CCHP’s Authorization Request Form.
- Non-Urgent Request: CCHP will notify the provider of the approval or denied decision within 14 days after receiving all necessary information.
- Urgent Request: CCHP will notify the provider of the approval or denied decision within 72 hours after receiving all necessary information.
