Advance Directives


You have a right to make decisions about your medical care, and a right to accept or refuse medical or surgical treatment. You also have a right to plan and direct the types of health care you may receive in the future if you become unable to express your wishes. An advance directive is a written document that describes your choices about the treatment you want or don’t want if you are unable to speak for yourself.

While CCHP is not involved in the implementation of a member’s advance directive, we respect our member’s wishes and do not discriminate based on choices that are outlined in advance directives.
