Professional Vision Services
Chorus Community Health Plans offers pediatric vision benefits through its administrator Professional Vision Services. To find a provider in the Professional Vision Services network, search the vision directory. Vision benefits for children 18 years old and younger include one routine eye exam and lenses (glasses or contacts) per benefit year, and one pair of eyeglass frames from the Pediatric Eyewear Collection every two years. Although adult vision services are not a covered benefit under Individual and Family Plans, Professional Vision Services will offer a discount to parents whose children receive services.
