CCHP cares about your health and well-being and wants to reward you for taking steps toward a healthier lifestyle. As one of our valued CCHP members, you are eligible to earn reward points by completing wellness activities. As you earn reward points, you can redeem those points for a gift card of your choosing.
As a current Medicaid member in Wisconsin, you are eligible to earn reward points by completing the following wellness activities:
All adult members (ages 18 and older)
- $25 for completing your annual health needs survey via the Wellness Portal
Adult members (ages 18 and older) with type 1 or type 2 diabetes
- $25 for completing your annual diabetic retinal eye exam
- Up to $100 for completing diabetes-related wellness activities
Pregnant/postpartum members
- $25 for enrolling in the Healthy Mom, Healthy Baby Program
- $25 for completing the Healthy Mom, Healthy Baby Program
- $40 for attending your postpartum visit (between 7 and 84 days after delivery)
