It's important to visit the doctor every year, even if you feel well. At your annual wellness visit, your primary care provider (PCP) can offer you screenings, vaccines, and tests to help you stay healthy. Here you will find more information on what a wellness visit is, what to expect, what to bring, how to schedule and more.

Getting an annual wellness visit is easy

  1. Find a primary care provider (PCP) in network. You can find a provider here , or call 1-800-482-8010.
  2. Make an appointment. You can call your primary care provider’s (PCP) office to schedule your visit. Tell them you want to make an annual wellness visit. It’s ok if your appointment is a few weeks or months from now. The important thing is to get it scheduled. To help you remember, you could plan it around your birthday or at the beginning of the year.
  3. Plan for your visit. You might need to take time off work or find someone to take care of your child, but your annual wellness visit is important for keeping you healthy. Below is a checklist of what to bring to your visit. If you need help with transportation, click here.

What is an annual wellness visit?

CCHP covers a wellness visit once a year at no cost to you. This visit helps you stay healthy and avoid preventable diseases.

During a wellness visit, a primary care provider (PCP) can:

  • Recommend screenings, vaccines and lab tests for your health needs.
  • Discuss ways to lower risk of developing health problems, even if you are in good health right now.
  • Help reach physical and mental health goals.


Frequently asked questions

An annual wellness visit helps check your health status. During the visit, your doctor discusses your past health and your current health. This visit is important so you and your primary care provider (PCP) can work together to keep you healthy or make you healthier.

Several types of providers can be used for preventive visits, such as:

  • Doctors in family medicine, pediatrics, internal medicine, obstetricians and gynecologists (ob-gyn)
  • Nurse practitioners (NPs) or nurse midwives
  • Physician assistants (PAs)

Yes! Having a primary care provider (PCP) can help you live longer and stay healthier since they are trained to look at every part of your health. Even if you see another doctor for other health needs, it’s still good to visit your primary care provider. They can find health problems before they get worse.


Each person’s visit will look different, but here is what you can expect.

  • Height, weight, and body mass index (BMI)
    • Your care team will take your height and weight. Body mass index (BMI) is a measure of your body fat based on your height and weight.
  • Blood pressure
    • Your care team will take your blood pressure. A normal blood pressure is 120/80 or less.
  • Physical Exam
    • Your care team will check things like your heart, lungs and ears to make sure they don’t see or hear any problems.
  • Vaccines
    • Your care team may offer you vaccines, depending on things like your age or your medical problems. Some vaccines, like a flu shot, are recommended once each year.
  • Cancer Screening for Adults
    • Your care team may offer screenings for various types of cancer based on things like your age or your family history.
  • Lab Work
    • Your care team may offer blood tests to check for things like your blood sugar, cholesterol, or kidney function.

Make sure you have the following with you at your visit:

  • ForwardHealth card (These cards are permanent, plastic, and have the word "ForwardHealth” on them)
    • To request a new insurance card:
      • Log into your ACCESS account
      • Call ForwardHealth member services at (800) 362-3002
  • Photo ID, such as a driver’s license
  • List of your current medications (prescriptions, over-the-counter medications, vitamins, supplements)
    • You can bring the bottles/containers with you.
