Visit CCHP staff at Midtown Clinic-Children’s Wisconsin

We are pleased to partner with Midtown Clinic-Children’s Wisconsin. We have CCHP staff on-site on Wednesdays from 9 a.m. – 3 p.m. to address concerns or questions you may have regarding your insurance, finding a doctor or dentist and how to receive help for any of your medical or behavioral health needs. As a reminder, Midtown Clinic is located at 5433 W. Fond du Lac Avenue, Milwaukee, WI 53216. No appointment is needed to meet with us in-person. In the event that you would rather call us for support or to find a time to meet that works best for you please call us at 1-877-900-2247. We hope to meet you in-person at Midtown Clinic!
